Medicare & Health Insurance in Texas: Your 2027 Guide

We publish 2027 county guides for 2 Texas counties. Across them, Medicare Advantage choice runs from 41 plans in Dallas County to 48 in Harris County, and 10 stand-alone Part D plans are sold statewide. People under 65 buy Marketplace coverage through HealthCare.gov.

Page last revised 15 October 2026. Plan data: CMS CY2027 landscape and 2027 Star Ratings.

Texas is the largest market we are licensed in outside Florida, and we serve it entirely by phone and video. The Marketplace mechanics are the same as Florida’s because Texas also uses HealthCare.gov, so the same income limits, deadlines and subsidy rules apply.

What changes is the scale of the need. Dallas and Harris counties each have an uninsured rate above 21% in the latest Census survey, higher than any single Florida county, and their 2027 Medicare Advantage shelves have a wider gap between the lowest and highest out-of-pocket limits than the First Coast.

10, compared with 12 in 2026. Monthly premiums run from $5.30 to $199, and the 2027 deductible can be no more than $700.

Through HealthCare.gov, the federal Marketplace, or a licensed agent. Open enrollment for 2027 coverage runs November 1, 2026 to January 15, 2027; enroll by December 15 for coverage that starts January 1.
No. KFF lists Texas as not having adopted the ACA Medicaid expansion as of October 2026. In states that did not expand, adults below 100% of the poverty line can fall into a gap where they earn too much for Medicaid but too little for Marketplace savings (HealthCare.gov).
Outside your six-month Medigap open enrollment, which starts the month you are 65 or older and have Part B, or a federal guaranteed-issue situation, a carrier may use medical underwriting (Medicare.gov). During open enrollment, Texas companies must sell you a policy even if you have health problems (Texas Department of Insurance).
October 15 to December 7, 2026. Changes made in that window take effect January 1, 2027.
