What you pay for Part B in 2026
Most people pay the standard premium of $202.90 a month in 2026. Most people have it taken out of their Social Security check automatically, so you may never see a bill.
You also pay the first $283 of Part B costs each year in 2026. After that, you usually pay 20% of the Medicare-approved amount for each covered service. Original Medicare has no yearly cap on that 20%.
| Cost | Amount |
|---|---|
| Standard monthly premium (2026) | $202.90 |
| Yearly deductible (2026) | $283 |
| Coinsurance after the deductible (2026) | Usually 20% |
| Highest monthly premium with IRMAA (2026) | $689.90 |
| 2027 premium | Not published as of October 15, 2026 |
When you pay more than $202.90
If your 2024 income was above $109,000 (single) or $218,000 (married filing jointly), you pay an extra charge called IRMAA in 2026. Your total Part B premium then runs from $284.10 to $689.90 a month. See what IRMAA is or run the IRMAA calculator.
A late sign-up also costs you. The Part B penalty adds 10% for each full year you could have had Part B but didn’t, and it lasts as long as you have Part B.
What about 2027?
CMS had not announced the 2027 Part B premium or deductible as of October 15, 2026. We will not guess it. When CMS posts the 2027 fact sheet, this page will show the new amounts.
If you pick a Medicare Advantage plan during Open Enrollment (October 15 – December 7, 2026), you still pay your Part B premium. If you’d rather talk it through, call 888-661-0126 and ask for a plain-language answer.
